Drug Screen Information

1. Order your drug screen online, this can be ordered as part of Package 3.
2. After you have completed your order, you will need to pick up a Drug Screen Chain of Custody Form from your school.
3. Contact one of the drug screen locations to schedule an appointment for your drug screen. Be careful NOT to use alternate or "Preferred" Quest Sites, as these will result in an additional collection fee.
4. Take your Chain of Custody Form with your Photo Identification to your appointment.
5. Your results will be reported directly to American DataBank within one to three business days of your drug testing. These results will be sent electronically to your school.
6. Please Note: If you do not use one of the approved drug screen facilities as provided in your packet or listed below, an additional collection fee of $12.00 will be charged.

Information

• Only use approved drug screen locations, any other sties will result in an additional fee to the applicant.
• DO NOT drink more than 8oz (a "glass") of fluid in the 2 hours before giving a uring smaple. An abundance of fluid in the body will result in a "dilute" reading, which constitutes a "flagged" result. It will keep you from attending class/clinical and requires IMMEDIATE re-payment and re-testing.
• If at the facility, and you are unable to produce a urine sample when requested, call American DataBank at 1-800-200-0853 for further steps.
• The lab will run extensive tests, and you may be contacted directly, to verify if the drug screen is negative/positive/dilute. Be sure to answer any calls you may get from our Medical Review Officer (MRO) as these will be an attempt to verify prescription information.
